BREAST ENLARGEMENT (breast implant, breast augmentation)

What is a breast augmentation?

This is a procedure where silicone/saline implants are put in behind breast tissue or chestwall muscles to enlarge the breasts. In suitable patients this will also provide some uplift of the breasts.

 

Other procedures can sometimes be combined, such as tightening of the breast tissue, tummy tuck, liposuction and arm reduction.

 

What are the types of breast augmentation?

There are many ways breast augmentation can be categorised.

 

- Type of implant All implants are made of a silicone shell. Most have silicone gel inside. Some can have saline inside where the size of the implant (and the breast) can be adjusted by changing the volume of the saline inside.

 

- Shape of the implant Most implants are round. Some patients may benefit from a teardrop (or anatomical) shaped implants.

 

Surface of the implant Most implants have a textured (or rough) surface. Some have a special chemical coating and are suitable in special situations.

 

- Type of gel and profile of the implant Most implants have a thick gel that do not easily leak out even if the implant ruptures. The implants can be high, medium or low profile, depending on what shape and size you wish for.

 

- Site of the incision Most operations are performed by placing the scars in the fold under the breast where they wont be noticeable. Rarely other incisions may be suitable.

 

Plan where the implant is placed The implant can be placed just under the breast tissue, or in very slim patients, under the chestwall muscles. Both techniques have their advantages and disadvantages. I perform the surgery in a way which combines the advantages of both techniques.

 

PIP implant: I do not use, and have never used PIP implants. If you have had a PIP implant in the past, or are unsure about the type of implant, I will be happy to advise.

 

Which type is suitable for me?

This will depend on the amount of breast tissue you have, any droopiness in the breasts, what size you wish to go up to and whether you have had previous breast surgery. I will discuss the different types with their advantages in detail with you at the consultation.

 

Details of the operation

Anaesthesia general anaesthesia, you will be asleep.

 

Type of incision As mentioned above, most patients will have a scar about an inch long in the fold under the breast.

 

Length of the operation about an hour.

 

Length of stay 1 night. Some patients feel comfortable enough to go home the same day. There may be a small drain on each side (tubes with attached bottles) to remove any blood/fluid collection.

 

Recovery

Recovery time is 2-3 weeks for light work and 4-6 weeks for full activity. You may need regular painkillers for the first week or two.

 

Possible complications

All surgical procedures carry a small risk of complications. For a breast enlargement operation the complications can be bleeding, infection, slow healing, lumpy scar and possibility of further surgery.

 

The long term risks include contracture (tightening) of scar tissue (capsule) around the implant which may distort the shape of the breast or change the position of the implant. There is also a low risk of rupture which will need removal/change of the implant.

BREAST REDUCTION

What is a breast reduction?

Some women are troubled by heavy breasts, sometimes after having children, but often since their puberty. The weight of the breasts can cause pain in the neck, shoulder and upper back because of poor posture. The skin can get sore under the breasts, particularly in warm weather. Losing weight often makes little or no change in the size of the breasts.

 

Breast reduction surgery removes the excess breast tissue and skin to reduce the weight and help with the discomfort.

 

What are the types of breast reduction?

There are different designs to remove excess breast tissue and skin, while maintaining some blood and nerve supply to the nipples.

 

Which type is suitable for you?

I will discuss this at the consultation. The type chosen will depend on the size of the breasts, amount of tissue to be removed and condition of the skin.

Usually 4-6 weeks for full normal activities.

BREAST UPLIFT (Mastopexy)

What is a breast uplift?

With time, and particularly after childbirth and breastfeeding, breasts may appear droopy as the skin loses elasticity. There also may be some loss of volume of breast tissue. An uplift is a procedure designed to tighten the breast tissue and lift the nipples to the appropriate position.

 

What are the types of breast uplift?

The breast position is improved by removing excess skin and/or by putting in a breast implant.

 

Which type is suitable for you?

This will depend on the position of the breasts, consistency of the breast tissue and also on how big/small you wish your breasts to be. I will explain the choices and have a detailed discussion with you at the consultation.

MALE BREAST CORRECTION (gynaecomastia)

What is gynaecomastia correction ?

Gynaecomastia is enlargement of male breast. The correction requires removing the fatty and gland tissue from the chest. This is done by a combination of liposuction and removal of the gland tissue through small scars. I offer a technique where all the scars are kept hidden in the armpit with no visible scars in the front of the chest.

Anaesthesia general anaesthesia, you will be asleep.

 

Type of incision In most patients the scars can be kept hidden in the armpits.

 

Length of the operation 1-2 hours, depending on the amount of tissue to be removed.

 

Length of stay usually 1 night. There may be drains (tubes with attached bottles) to remove any blood/fluid collection.

 

Recovery

Return to work and light activities usually 2-3 weeks.

 

Return to full activities usually 3-4 weeks.

 

Return to work usually 2-3 weeks, provided it does not involve any heavy lifting.

CORRECTION OF NIPPLE (Inverted Nipple & Nipple Reduction)

When is correction of nipple needed?

Inverted nipple (where the nipple does not project out of the breast) can be corrected by dividing the ducts and bands that hold the nipple down and by creating a tissue bridge under the nipple to hold it up.

 

This is all done with minimal scarring. An enlarged nipple can be reduced to normal size without damaging the blood supply or nerve supply, again with minimal scarring.

 

An enlarged areola (the pigmented skin around the nipple) similarly can be reduced in size.

 

Details of the operation

Anaesthesia local anaesthesia, with numbing injection.

 

Type of incision The scars are kept within the areola (the pigmented skin around the nipple) where they are less noticeable.

 

Length of the operation 30 minutes.

 

Length of stay none, you will be able to go home straight after the procedure.

 

Recovery and aftercare

Return to light activities straight away.

 

Return to full activities, including going to the gym/jogging two weeks.

 

Return to work within 2-3 days for light work.

 

You will need to have a special dressing for the first few days. I will arrange for a change of dressing at one week and see you typically at four weeks.

 

You will also need to look after the scar to help it settle for the first few months I will give you detailed instructions about this.

 

Possible complications

All surgical procedures carry a small risk of complications. For correction of nipple, the complications can be bleeding, infection or slow healing. A corrected inverted nipple has a small chance of inverting again.

BREAST ASYMMETRY

Asymmetry between breasts by size or shape is common. This can be corrected in a number of ways depending on the nature of the difference. The common techniques used are tightening/reduction of the larger breast and/or putting an implant in the smaller breast. Sometimes different sized implants are used in both breasts to improve symmetry. In younger women an expander implant (where size can be adjusted over a period of time by injecting saline into the implant) can be very helpful.

 

I will discuss the different types of techniques with their advantages in detail with you at the consultation.
